The office does not accept medical patients for walk-in appointments or grant appointments to patients who walk in with referrals. All referrals are reviewed by our clinic coordinator and board-certified dermatologists and then triaged for booking prior to an appointment being provided.

 

Please note: referrals sent to the office are reviewed and triaged before being accepted and scheduled with our dermatologists. Referrals declined are returned to the referring physician. If applicable, the referring MD is provided with suggestions of where the referral may be directed instead.

Our office accepts medical dermatology referrals for patients aged 16 and older.



Medical/ OHIP Appointment Policy

1. To book medical-related appointments with the dermatologist, a valid OHIP card is required. Your OHIP card is also required on the day of the appointment, whether conducted in-person or virtually.

Please note, OHIP appointments are prioritized for patients within DermAtelier on Avenue’s Home and Community Support Services Catchment Area (formerly known as LHIN – Local Health Integrated Network), with the exception of patients referred for suspected skin cancer. OHIP referrals originating outside of this zone are redirected to the referring provider for consideration of care closer to the patient’s home address and for feasibility of in-person follow-up.

2. Your appointment is confirmed via a phone call with you.  Be sure to check phone voicemail, email, text for appointment confirmation reminders.  Each patient will receive 2 appointment reminders (e.g. email), in addition to the initial verbal contact made with you for your appointment.

Please note, the appointment reminders sent are a courtesy provided by the clinic.  It is each patient’s responsibility to know their dermatology appointment date and time, as confirmed during your phone call. 

3. For your OHIP medical appointment, please allow up to one hour (1 hour/ 60 minutes) in the office.  This time includes your intake, form completion, assessment by dermatologist, and possibly same-day procedure.  Please allot time to obtain parking prior to your scheduled appointment (See parking suggestions here)

4. Please bring any prescribed medications that were already used for your skin condition.

5. To discuss your concern with sufficient detail, including all available treatment options, the appointment will focus on the Issue for which you are Referred.  If necessary, other concerns may be addressed with a referral at a separate appointment.

6. To manage the flow of patients and minimize reception room crowding, patients arriving after their scheduled appointment time may be asked to rebook that day’s appointment and arrive on time at the next appointment.

7. Family members accompanying a patient who request an appointment (and who do not have a previously booked appointment) may obtain a referral to be seen at an alternate, scheduled appointment time.

8. Due to infection control limitations, we are unable to accommodate pets or animals of any kind in the patient exam and procedure rooms, with the exception of service dogs.

9. Please notify our office at least 24 hours in advance if you need to cancel or reschedule your medical-related appointment

Missed appointments, Cancellations and Appointment Rescheduling less than 24 hours in advance of your appointment require a re-booking fee as follows prior to your appointment being rescheduled:

MEDICAL/ HAIR LOSS ALOPECIA APPOINTMENT POLICY

How do I see a dermatologist for my hair loss?

At DermAtelier on Avenue, we understand that hair loss, or “alopecia” is a very upsetting condition to experience. In certain cases it is lifelong, without known cures, which can add to feelings of distress and disappointment.

Patients seen in consultation by our dermatologists are referred by their primary care provider (Family physician or Nurse practitioner) in order to facilitate shared care, as well as in anticipation of patients receiving prescription renewals on an indefinite basis. Our goal is to provide each patient with a clear diagnosis of their hair loss. We also offer medical and cosmetic treatment regimens tailored to you.

Patients with hair loss deemed non-scarring, are monitored for up to a 3 – 6 month period if oral prescription medication is introduced. Patients with scarring forms of hair loss are monitored for up to a 6 – 12 month period if oral prescription medications are administered. The office then provides a summary of care with treatment plan for the referring physician to follow going forward, and each patient is discharged. This policy applies to all patients and helps patients receive diagnoses and dermatologic care, while allowing appointment capacity for other patients with hair loss to be seen in a timely manner.

HOW DO I PREPARE FOR MY HAIR LOSS APPOINTMENT?

We want to optimize your hair loss assessment with our dermatologists. In advance of your hair loss appointment with our dermatologists it is requested that you:

  • Shampoo scalp within 2-7 days of the appointment.
  • Come to clinic without any hair / cosmetic products applied to the scalp.
  • Your lab results from your primary care provider were sent to us in advance. Please be sure to bring any updated hair specific labs with you (TSH, Vitamin D, B12, Ferritin)
  • Remove any installed hair style/ hair style with added hair (sew-in weave, lacefront weave, clip-in extensions, sew-in extensions, box braids, crochet braids, passion twists, etc) to allow full visualization of the scalp.

*Individuals who are unable to remove their hairstyle will be rebooked to the earliest available appointment when they will be able to have the hairstyle removed.

Cosmetic/ Private-Pay Appointment Policy

 

 

1.  To book cosmetic appointments with the dermatologist, you may contact the office by phone, email, or in-person visit.  OHIP does not pay for therapy or treatment of “non medically necessary” conditions or concerns. This includes, but is not limited to, removal of regular moles, removal of skin tags, cyst excision, removal of age-spots (e.g. seborrheic keratoses), injection of neurotoxin or injection of volume-enhancing filler.

2. Your appointment is confirmed via a phone call with you.  Be sure to check phone voicemail, email, text for appointment confirmation reminders.  Each patient will receive 2 appointment reminders (e.g. email), in addition to the initial verbal contact made with you for your appointment.

Please note, the appointment reminders sent are a courtesy provided by the clinic.  It is each patient’s responsibility to know their dermatology appointment date and time, as confirmed during your phone call. 

3. Please review all information and consent forms prior to your appointment.  This provides you with the ability to gather questions you may have for Dr. Beach that day. 

4. Patients booked for treatment after an initial cosmetic appointment may rebook to discuss conditions and treatment.  These secondary discussions are billed privately at a starting rate of $100 + HST.

For procedure appointments in which no treatment is done / the time is limited to discussion, fees and private, non-OHIP and billed to you at $100 + HST. Payment is due on the day of the appointment.

5. Cosmetic patients receiving injectables (neuromodulators, volumizing HA or calcium hydroxylapatite filler) and treatments (e.g. benign lesion treatment) will be informed at their appointment of the advised check-in interval.  These appointments are part of the service to ensure your results are complete. Generally this timeframe ranges from 2 weeks for Neuromodulators (BOTOX® or Dysport®,  Xeomin® or Nuceiva®) and 2- 4 weeks for filler, and 3 – 4 weeks after treatment of benign lesions. Patients returning to receive treatment outside of these advised time frames will be assessed in person and billed as a new treatment. 

6. Payment is due the same day upon completion of the service. For all cosmetic/ non-OHIP procedures, please pay by debit, Visa, American Express or MasterCard, or Interac e-transfer only. DermAtelier on Avenue is a cashless office and has no cash on site.

7.To manage the flow of patients. minimize reception room crowding and ensure adequate time for your procedure, patients arriving after their scheduled appointment time may be asked to rebook their appointment and arrive on time at the next appointment.

8. Due to infection control limitations, we are unable to accommodate pets or animals of any kind in the patient exam and procedure rooms, with the exception of service dogs.

9. Please notify our office at least 24 hours in advance if you need to cancel or reschedule your cosmetic appointment, or a $150 fee applies.

10. Patients who require completion and signature of insurance, employment, or legal forms, or reply to extensive patient inquiries, are billed $75, payable via debit/ credit card/ e-transfer or via online payment at shop.dermatelieronavenue.com
